Useful Forms
If you want to save time at logistics, or if you need to fill out a form in advance and send it along with someone, you can probably find that form here. Please feel free to download and print it, then bring it with you when you come. We will also have plenty of copies of these at logistics when you check in, so no fear if you forget!
- Legal Release - All ages. This form is required for all participants in our game before they begin playing. Minors (under 18) must have this signed by a parent or guardian.
- COVID Release - All ages. This for is required for all participants in our game before they begin playing. Minors (under 18) must have this signed by a parent or guardian.
- Image Release - All ages. This is an optional form giving permission for the national organization to use your image on our website/promotion materials/etc. Portland's legal release (first link above) covers this for our chapter. Minors (under 18) must have this signed by a parent or guardian (if desired).
